MINNESOTA STATE HIGH SCHOOL LEAGUE (MSHSL) FORMS

Step 1: Read MSHSL Brochure 2024-25 (pages 1-9)

Step 2: Complete & submit MSHSL Eligibility Forms 2024-25 (pages 10-12)

Step 3: Complete Sports Physical Exam Form 2024-25

Physicals are good for 3 years from the date of examination. A sports physical is required to be on file with the Activities office before a student can participate in practice or activities.

Forms can be submitted in the following ways:

• print & drop off at the PRHS office
• mail to Pelican Rapids High School, PO Box 642, Pelican Rapids, MN 56572

TRAVEL WAIVER FORM

Sport fees apply to all athletics (except Fishing League & Trap Shooting, which have their own fees).
Activity fees apply to Knowledge Bowl, Math Teams, Speech, & Theatre.

The Application for Educational Benefits form must be completed each school year before reduced or free sports/activity fees will apply.

Activity tickets can be used for regular season games only

• Wrestling

We prefer that all fees are paid online using SchoolPay.

If you are unable to pay online, Sports/Activity fees can also be paid in the following ways:

• check payable to ISD 548
• cash (drop off at PRHS school office)
